For Your Health

Do you have patients who need help and encouragement to follow your treatment plan for their chronic condition, smoking cessation efforts, or high-risk pregnancy? Rocky Mountain Health Plans For Your Health can help!

The guidelines in each area describe an evidence-based approach to care management. Each physician still must use his/her discretion and independent medical judgment in caring for patients based on the patients’ own medical condition and needs.

You have permission to download and copy any of the materials listed below.

CHRONIC CONDITIONS

Evidence-based guidelines and tools will help you develop a treatment plan for your patients who have asthma, cardiovascular disease, depression, or diabetes. RMHP encourages our members to manage their chronic condition by following their doctor’s treatment plan, becoming educated about their disease, and learning what affects their condition.

If you have an RMHP member with a chronic condition that you would like to refer for chronic disease health education or case management, contact a health educator at 970-248-5106 or 800-843-0719, ext. 5106.
